The Registered Nurse Transition Services Community Coordinator - ALC Program (Sarcee ALC, Calgary) screens, coordinates and evaluates the needs of patients and families who cannot function independently in the community. The Coordinator expedites discharges for individuals who require community services. This involves extensive assessment, facilitation and negotiation to develop the plan of discharge and complete the electronic community referral for Continuing Care Living Options. This position requires significant walking (including stairs and long hallways) of a minimum of 3-5 km daily, repetitive adjustment and exertion including sit/stand/bending throughout shift, repetitive keyboarding and written documentation for a minimum of 6 hours and carrying, lifting and pulling resource materials between 20-25 lbs. Able to perform physical requirements such as being on ones feet for an extended period (walking and standing). Must be able to provide physical assistance to clients in continuing care setting and physically able to intervene to ensure client safety. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).Additional Required Qualifications: Minimum 3 years nursing experience required. Knowledgeable with programs PARIS, Strata pathways. In addition, the incumbent will be expected to demonstrate proficiency in the following areas: Demonstrated knowledge of community agencies and resources in order to facilitate the linkage to appropriate services. Demonstrated conflict resolution, problem-solving, negotiation and facilitation skills. Excellent organizational and time management skills to efficiently organize and prioritize workload. Ability to be self-directed and exercises independent decision making, nursing judgment and critical thinking skills. Excellent interpersonal and communication skills to effectively work with a multi-disciplinary team of Physicians, Nurses and Allied Health professionals. Actively encourages and integrates client and family centered care principles within the nursing team and own nursing practice. Ability to provide comprehensive assessment skills of individuals within the context of their physical and social environment. Ability to monitor financial accountabilities related to care delivery. Shown commitment to maintaining knowledge and skills through professional development or continuing education. Excellent written, verbal and computer processing skills to appropriately capture pertinent client information.Preferred Qualifications: Previous nursing practice in community and in the geriatric sector preferred. Minimum 3 years community experience (IHC, ISFL) and or current/previous Transition Services Coordinator. Strong conflict resolution and change management skills are preferred. Strong computer skills and ability to navigate and data entry into a variety of software programs some of which include PARIS, Strata Health programs, Sunrise Clinical Manager and upcoming Epic/Connect Care.
